Local AI for Therapists and Counselors: Writing Without Sharing Session Notes

Therapists write constantly: session notes, treatment summaries, letters to GPs and insurers, appointment emails, referral requests. Most of it contains exactly the kind of information that should never leave the room.

That makes the usual advice about AI writing tools (“just paste it into a chatbot”) a non-starter. A cloud service, however well run, means a copy of a client’s mental health information sitting on someone else’s servers under someone else’s retention policy. In most jurisdictions that is health data with the strictest handling rules there are.

A local model changes the question. If the rewrite happens on your own laptop and nothing is transmitted, the text never leaves your control.

The Writing That Benefits Most

Not every piece of clinical writing should involve AI. The useful cases are the ones where you already know what you want to say and the problem is phrasing, length, or tone.

Letters to other professionals. A referral letter or a summary for a GP needs to be concise, clinical, and readable by someone with four minutes. Turning your draft into that register is exactly what a rewrite does well.

Client emails about logistics. Rescheduling, fees, cancellations, holiday cover. These need to be warm without being over-familiar and firm without sounding cold.

Plain-language explanations. A short explanation of what a treatment approach involves, for a client who asked.

Where AI Should Not Be Used

Be clear about the limits.

  • Clinical judgment. A model has no access to the relationship, the history, or what you noticed in the room. It should never suggest a formulation, a diagnosis, or a risk assessment.
  • Risk documentation. Safeguarding records and risk assessments should be in your own words, contemporaneous, and defensible. Editing them for style risks changing what they say.
  • Anything you have not read in full afterwards. A rewrite that drops a qualifier (“reports occasional” becoming “reports”) changes clinical meaning.

Your professional body’s ethics code still applies. The American Psychological Association’s Ethics Code, for example, places confidentiality and competence at the centre, and the same principles appear in counselling codes elsewhere. The second version gives the client concrete options, keeps the apology, and avoids the breathless tone that can unsettle someone already anxious about change.

Setting Up A Private Workflow

What About Note-Taking Apps With AI?

Several practice management systems now offer AI note generation from session audio. Those are a different category: they record the session, which needs explicit client consent, and they process audio in the cloud. Whether they are acceptable depends on your jurisdiction, your contract with the vendor, and your clients’ informed consent. Common Questions

Is using a local AI model compliant with HIPAA or GDPR?

A model that runs entirely on your own device does not transmit data to a third party, which removes the business associate or processor question for that step. You still need the usual safeguards for the device itself, such as encryption and access control, and you should check your own professional and legal obligations.

Can I use AI to write session notes from scratch?

You can use it to tidy your own dictated or rough notes, but the clinical content should come from you. Generated notes that you did not observe are inaccurate records, whatever tool produced them.

Do I need to tell clients I use AI to edit letters?

Many practitioners mention it in their privacy notice or intake paperwork. If the processing is local and nothing is shared, the confidentiality risk is low, but transparency is still good practice and some professional bodies expect it.

What if my laptop is shared with family?
